The role of a Carers Champion is to promote awareness, identification, and registration of Adult and Young Carers, listen to Carers needs, signpost to appropriate services and to help coordinate actions for Carers within the Practice to support the needs of the Carer and their loved ones.

Patients:

If you are involved in any level of caring responsibilities, consider registering yourself with your GP Practice Carers Champion

A Carers Champion along with the wider Practice Team can support Carers by:

  • Ensuring that all Staff understand your caring role

  • Offer you flexible appointments to suit your caring needs

  • Offer you priority Flu vaccinations and Health Checks

  • Involve you in planning the care of the person you look after

  • Signpost you to useful information and support

  • Gain consent to share information and make referrals for support on your behalf
